Asthma: If your child has asthma, please complete the following three forms and file them with the Center.  For your convenience, we also provide you with a checklist of the asthma forms (also in Word format).

• Written order from an authorized prescriber (or in Word format)

• Check list for Health Care Provider Medication Form (or in Word format)

• Individualized health care plan (asthma) (or in Word format)
